The most frequent cause of this is that we wish to edit and algorithm definition and we press [Ctrl]+[D] in page mode before opening the question for editing.  (Recall, EVP uses the command [Ctrl]+[D] to duplicate questions in page mode and [Ctrl]+[D] to open the Edit Algorithm Definitions box in question mode).

The solution is to immediately delete the active (duplicated) question.

• Press the [Delete] key.  A Caution box will open.

• Click on Yes.
